Healthy Roster App
As part of our commitment to keeping your student-athlete safe, healthy, and supported, we are implementing the Healthy Roster app. This platform will allow me to communicate directly with you regarding any injuries, medical documentation, or next steps if care is needed.
With Healthy Roster, you will:
• Receive immediate notifications if your athlete is injured during practice or competition.
• View updates on evaluations, treatment plans, and recovery recommendations.
• Stay connected with me as the athletic trainer for seamless coordination of care.
Having this app helps us ensure that you are informed right away and can take the proper steps for your child’s care without delay.

Healthy Roster App: Step-by-Step Setup Guide
1. Watch for Email Invitation
• Check your email for a message from noreply@healthyroster.com with the subject “Join Notre Dame Preparatory on Healthy Roster.”
• If you do not receive the email within one week, contact the athletic trainer at trainer@ndpsaints.org.
2. Download the Healthy Roster App
• In the email, use the link to download the Healthy Roster App or go to your device’s app store (App Store for Apple, Google Play for Android).
• Search for “Healthy Roster” and download the app. It will be an orange logo with "H+R".
• Enter the unique access code found in the email (each child’s code is different).
• Select “Sign up.” Create one account for all your children using the same email provided earlier—each will be linked to your account.

3. Complete Account Setup
• Enter your personal information as prompted.
• Create a secure password (at least 8 characters, mixed case, and a number/special character).
• Read and accept Healthy Roster’s Terms and Conditions.
• Choose and answer three security questions and save answers.
4. Log In and Verify Your Athlete’s Profile
• Open the app and log in with the email and password you created.
• Your student-athlete’s profile should appear. If anyone is missing, contact the athletic trainer.
• Update important information such as emergency contacts, insurance, allergies, health conditions, and medications. Make sure at least two emergency contacts are listed.
5. Upload Required Documents
• Locate your athlete’s profile in the app.
• Click on “Documents” to access, fill out, or upload any required forms (medical history, insurance, consent, etc.) as directed by your school.
6. Stay Informed
• You will now automatically receive notifications from Healthy Roster if your athlete experiences an injury, if forms are missing, or when action is needed.
