Rehab Plus will host its annual Sports Physical Day on Saturday, April 12 from 7 a.m. to 11 a.m. The cost of the physical is $25 which will be donated back to Notre Dame Prep by Rehab Plus. 

All student-athletes interested in participating in Notre Dame Prep athletics during the 2025-26 school year, MUST have an updated physical dated AFTER March 1, 2025, per AIA regulations. NDP ENFORCES THE SAME PHYSICAL REQUIREMENT FOR NON-AIA SPORTS. This is an easy and convenient way to take care of your sports physical early! 

Please have your sports physical medical history form filled out before you arrive. You can download a copy of the physical form here. It is eight pages long. 

 
Directions for using this online form:
  1. Fill out parent information completely
  2. Select your preferred appointment time
  3. Enter the number of students (attendees) you would like to sign up for your desired appointment time(s)
  4. Click on "Add Attendees"
  5. In the fields that appear, fill in the student(s)' first name, last name and grade for the 2024-25 school year
  6. Proceed to payment after you've completed the information for each attendee.
