Athletics Registration

Download the AIA Pre-Participation Physical Examination Form and bring it to your appointment.

Important: Physicals must be completed after March 1, 2026, to be valid for the upcoming athletic season.

Register through FinalForms by selecting the Parent icon and clicking New Account.

Parents will need to:

• Create an account using their name, date of birth, and email address.
• Confirm their account through email.
• Add and register their student-athlete.
• Select all sports in which their students plan to participate.

The following documents must be submitted through FinalForms:

• Doctor-signed AIA physical form.
• Medical history and health information.
• Medical insurance information.
• Brainbook certification (first-year and transfer students only).
• Opioid course certification (first-year and transfer students only).
• Anti-Hazing course certification (required for all students).
• Transportation of Minors form (no longer require notarization).

Parents/guardians and student-athletes must review and electronically sign all required athletic policies and agreements.
