Athletics

To participate in sports in the 2026-27 school year, all athletes must register online to be cleared and approved before the first tryout or practice. The NDP Athletic Department is using a secure registration platform called, Final Forms. This site provides our families with a safe, easy, and user-friendly way to register for Notre Dame Prep athletics.

The day after the school year ends, Final Forms will automatically reset and clear the previous year's forms. Both student athletes and parents must go into Final Forms and electronically sign all online forms for the new year.

Please note that the new transportation form does not require notarization.

Registration Process: 

1. Download the AIA Pre-participation Physical Examination Form

Take this form with you to your physical; the exam date must be after March 1 of the upcoming athletic season

2. Register Now for the all sports season through Final Forms. Locate the parent icon and click New Account. Register with your name, date of birth and email and confirm your account via account confirmation email. Then Register Student. Multi-sport athletes must click the checkbox for each sport

3. FinalForms will require you to:

• Upload ALL of the pages of the the doctor-signed AIA pre-participation physical form 
• Fill-in your medical history and health information, medical insurance provider and policy information 
• Upload your Brainbook, Anti-Hazing, and Opioid Course Certification (required for first year and transfer students only) 
• Upload the Transportation of Minors form

4. Accept policy agreements with electronic signatures by Parent/Guardian and Student Athlete

NDP is now a proud partner with GoFan for all of NDP's Game Day Tickets. You will be able to find all of our home game tickets on GoFan as well as all of our away games if the hosting school is also a partner with GoFan! We hope this new partnership will make purchasing tickets easier for our NDP Community as well as our guests!

Tickets are needed for the following sports:

• Football (varsity, JV, and freshman)
• Volleyball (1 ticket per night, admission includes freshman, JV, and varsity matches)
• Flag football (varsity)

We charge the following for all home games: 

• Adult - $7 price + $1 fee
• Non-NDP Student w/ ID - $5 price + $1 fee
• Military & Seniors - $5 price + $1 fee
• NDP Students with ID - free
